What It Measures

Free thyroxine, or free T4, measures the unbound fraction of thyroxine circulating in your blood, the portion that is not attached to transport proteins and is therefore available for your body to convert into the active thyroid hormone T3. Because it is unaffected by fluctuations in binding protein levels, free T4 is widely regarded as one of the most reliable markers of thyroid function and is routinely measured alongside TSH to give a clear and accurate picture of how your thyroid is performing.

You can think of free T4 as the readily accessible supply in your body's thyroid hormone system. While total T4 includes hormone that is bound up and essentially on hold, free T4 represents the fraction that can be immediately taken up by your tissues and converted into T3 wherever it is needed, whether that is your liver processing energy, your brain maintaining concentration, or your muscles supporting physical activity. This makes free T4 a particularly practical measurement, as it reflects the hormone that is genuinely available to do its job rather than the total amount in circulation. When your thyroid is working well, free T4 levels remain stable and your tissues receive a consistent supply of raw material for T3 production.

Your free T4 levels can be influenced by a variety of factors. Changes in thyroid function are the most direct cause of shifts in free T4, but other elements play a part too. Certain medications, including levothyroxine, amiodarone, heparin, and some anti-seizure drugs, can affect your results. Severe illness, sometimes referred to as non-thyroidal illness, can temporarily lower free T4 as the body adjusts its metabolic rate during recovery. Biotin supplements, which are popular for hair and nail health, can interfere with some laboratory assays and produce misleading results, so it is worth mentioning any supplements you take when discussing your results. Pregnancy also shifts thyroid hormone dynamics, with free T4 requirements increasing during the first trimester. If your free T4 level is higher or lower than expected, your GP will interpret it alongside your TSH and clinical symptoms to understand whether your thyroid needs any additional support or monitoring.

Above Range

A higher-than-expected free T4 level may indicate that your thyroid is a little more active than usual, releasing more thyroxine into your bloodstream. This can sometimes be temporary and related to factors such as medication changes or mild thyroid fluctuations. Your GP can assess this alongside your other thyroid results and symptoms, and will advise whether any further monitoring or investigation would be helpful.
