What It Measures

Thyroid Peroxidase Antibodies (TPO antibodies) are proteins produced by your immune system that are directed against thyroid peroxidase, an enzyme that plays an essential role in the production of thyroid hormones. Your thyroid is a small, butterfly-shaped gland at the front of your neck, and it relies on TPO to help convert iodine from your diet into the hormones T3 and T4, which regulate your metabolism, energy levels, body temperature, and much more. When TPO antibodies are present at elevated levels, it suggests that the immune system may be interacting with the thyroid gland in a way that could affect its function over time. Understanding your TPO antibody level is important because it provides information that standard thyroid function tests, TSH, Free T4, and Free T3, cannot offer on their own.

While those tests tell you how the thyroid is performing right now, TPO antibodies can indicate whether an autoimmune process may be at work behind the scenes, sometimes years before any change in thyroid hormone levels becomes apparent. Think of it as the difference between checking whether your car is running smoothly today versus looking under the bonnet to see if anything is slowly wearing down. The presence of TPO antibodies does not automatically mean your thyroid will stop working properly, many people with mildly elevated levels maintain perfectly normal thyroid function for their entire lives, but it does flag that closer monitoring may be worthwhile. TPO antibodies are more common in women than men and tend to become more prevalent with age.

Genetic predisposition is a significant factor, if close family members have thyroid conditions, you may be more likely to have elevated levels. Other influences include smoking, high iodine intake, certain medications, stress, and hormonal changes such as pregnancy or the menopause. Selenium and vitamin D status have also been studied in relation to thyroid antibody levels, though evidence is still developing. If your TPO antibody level is elevated, your GP can advise on an appropriate monitoring schedule for your thyroid function and discuss any steps that might help support your thyroid health over the long term.

High

Above Range

Raised TPO antibodies indicate that your immune system is producing antibodies that target your thyroid gland, a pattern associated with autoimmune thyroid conditions such as Hashimoto's thyroiditis. While many people with elevated TPO antibodies maintain normal thyroid function for years, there is an increased likelihood of developing thyroid underactivity over time.
