What It Measures

Creatinine is a waste product generated by your muscles as a natural by-product of energy use. It is produced at a fairly constant daily rate and cleared almost entirely by the kidneys, which is what makes it such a widely used indicator of kidney filtration. Along with urea and eGFR, creatinine is part of every routine kidney function panel. Its real value, however, is not in the raw number alone but in how well the number fits the person producing it.

You can think of creatinine as an exhaust fume from muscle activity. Creatine, the compound your muscles use to fuel short bursts of power, breaks down into creatinine after use. Because the input is steady, the level in your blood reflects how fast your kidneys are removing it. Your kidneys contain millions of tiny filtering units called nephrons that clear creatinine continuously. If filtration slows, creatinine rises, if filtration is efficient, it stays low. The twist is that how much creatinine your body makes varies a lot between people, and that variation is what confounds this marker.

Baseline creatinine tracks closely with muscle mass. Muscular people, bodybuilders, rugby players, anyone who lifts seriously, naturally run higher, often at or above the top of the reference range, without any kidney problem. Creatine supplementation does the same, as does a diet very high in red meat, intense resistance exercise in the days before a blood test, and dehydration. Women and older adults typically sit lower because of lower average muscle mass. This is why creatinine-based eGFR can misleadingly look reduced in an otherwise healthy athletic person. When that happens, Cystatin C is the much better next test: it is produced at a constant rate by nearly every cell in the body and is not affected by muscle mass, diet, or exercise, so it gives a cleaner read on true kidney filtration.

If your creatinine is raised, your GP will put it in context, your build, recent exercise, supplementation, hydration, and other kidney markers. In many cases the interpretation is reassuring. Where the picture is genuinely unclear, or where creatinine stays persistently high on repeat testing, Cystatin C and follow-up kidney assessment are usually the sensible next steps.

Below Range

A low creatinine level can reflect naturally smaller muscle mass or a lighter build. It can also occur in pregnancy, when blood volume increases and creatinine is diluted. If your other kidney markers are within range, this finding does not typically require further action.

Normal

Within Range

Your creatinine level is within the healthy range, which is a reassuring sign that your kidneys are filtering waste products effectively. Alongside eGFR, this is one of the key markers used to assess kidney health.

High

Above Range

A raised creatinine level means there is more of this muscle-derived waste product in your blood than expected. Common non-kidney causes include higher muscle mass, creatine supplementation, a diet high in red meat, intense exercise before the test, or dehydration. A test called Cystatin C gives a cleaner picture in these cases as it is not affected by muscle mass. Your GP can review this alongside your eGFR and lifestyle context.
