Your body moves iron around on a protein called transferrin, and TIBC essentially tells you how many empty seats are available on the transferrin bus.

Your body moves iron around on a protein called transferrin, and TIBC essentially tells you how many empty seats are available on the transferrin bus. When your iron stores are low, your body responds by making more transferrin, creating more seats, in an effort to capture every bit of iron it can find. This means TIBC rises when you are iron deficient. Conversely, when iron is plentiful, fewer transport seats are needed, so TIBC falls.
This makes TIBC a particularly useful companion to serum iron and ferritin. On its own, a ferritin level can be misleading because ferritin rises during inflammation, infection, or liver problems regardless of your true iron status. TIBC helps cut through that noise. A high TIBC alongside low serum iron strongly points to genuine iron deficiency, even when ferritin looks normal. TIBC is also used to calculate transferrin saturation (TSAT), which expresses how much of the available carrying capacity is actually being used. Together, these markers paint a complete picture of iron supply and demand in your body.
Low TIBC can occur with chronic inflammation, liver disease, or conditions where the body is overloaded with iron. High TIBC is most commonly seen in iron deficiency and during pregnancy, when iron demands increase. If your TIBC is outside the expected range, your GP can advise whether dietary changes, supplementation, or further tests are appropriate.
